You understand by viewing the Community Thrive app that you will be introduced to information about improving mental health literacy for caregivers who provide care for an elderly family member with dementia in the Chinese language. Information in the app focuses on: 1) Normalizing caregiver stress and burnout through education on caregiver burnout and awareness of associated symptoms; 2) Increasing perceived social support and engagement in activities that can reduce stress; and 3) Helping individuals seek and access support resources in the San Francisco Bay Area. The App provides caregiver support through information, guidance and links to resources in 3 domains:

1. Signs and Symptoms Awareness: Providing information and tools to develop greater self-awareness of burnout and compassion fatigue related to caregiving;
2. Motivational Guidance: In the form of inspirational messages and stories as well as suggestions for self-care activities that help to normalize caregiver stressors; and
3. Resource Sharing: Sharing of critical information and resources to support users who need access to behavioral health and social services, as well as caregiver training and education.

Privacy Policy
KCCEB values its clients, users and members (collectively ‘clients). Although it is necessary that we obtain accurate and current information about our clients in order to provide the highest level of service, we are dedicated to protecting the privacy and confidentiality of our clients’ information.

This privacy policy applies to current or former clients who register for one of our apps. Throughout the policy, we refer to information that personally identifies you or your accounts as "personal information."

KCCEB will never sell your personal information to anyone. Other than the special cases listed in this policy, we will not use or distribute your personal information in any way without prior notification and consent from you. We promise that we will uphold the privacy policies and procedures as set forth below.

By submitting your information into any web form on our app, you are consenting to allow us to collect, store, and process your personal information, which we may use to send you a newsletter. Additionally, by entering your phone number into one of the web forms, you are agreeing to possible phone calls by a KCCEB representative.

Information we collect about our clients on the app

Information sharing or distribution

• KCCEB will not sell personal information regarding our current or former clients to anyone.
• KCCEB will not share personal information regarding our current or former clients with anyone outside of KCCEB except as outlined below.
• KCCEB will aggregate user data and share it in an aggregate form with certain local or state governmental bodies for funding and evaluation purposes. By using the app, users agree to KCCEB aggregating and sharing such aggregated data with local or state governmental bodies.
• KCCEB may enter into a “strategic partnership” with a service provider if we believe that our customers might be interested in hearing about its products or services. If we do, we will make its services available to you from within the Community Thrive app site. By clicking on a logo or special placement of that service provider, you agree that we may contact that service provider to assure its representation to you is in your best interests. In the event we enter into a strategic partnership agreement, we will only share limited information such as your name and email address, and only if you initiate contact with our partner by clicking on the partner’s logo or special placement. We will require that the partner only use the information provided for responding to your inquiry.
• We may disclose information about current or former clients in order to cooperate with legal or regulatory authorities or pursuant to a court order or subpoena. For example, we may be required to disclose personal information to cooperate with regulators or law enforcement authorities, resolve consumer disputes, perform credit/authentication checks, or for institutional risk control.
• We may also disclose personal information as necessary to collect debts, enforce our legal rights or otherwise protect our interests and property.
• We continue to evaluate our efforts to protect personal information and make every effort to keep your personal information accurate and up-to-date. If you identify any inaccuracy in your personal information, or you need to make a change to that information, please contact us so that we may promptly update our records.

Opt-in and opt-out
By creating an account on our mobile app, you agree to the terms and conditions, thereby opting in to receive text messages from our platform. These messages will be sent via a third-party service provider named Twilio. Should you wish to opt out of receiving these messages in the future, you can do so by deleting your account through the profile section of the app. Please note that maintaining an active account necessitates consent to receive these text messages; failure to provide such consent will result in the inability to hold an active account. However, users can still access sections of the app that do not require account creation or login.

Information security
Because KCCEB is committed to the protection of your personal information and to protecting your privacy, only authorized KCCEB employees or agents will have access to your personal information. Customer account passwords are encrypted and not retrievable as clear text from our systems by anyone. All of our employees, affiliates and service providers are held to the highest standards of privacy and security.

Right to Terminate Use and have Information removed from Records Community Thrive app users may terminate their use of the Comunity Thrive app at any time and may at any time ask to have their information removed from KCCEB’s records by emailing KCCEB at general@kcceb.org. Furthermore, users have certain rights under the General Data Protection Regulations, listed below.

General Data Protection Regulations (GDPR)

• In accordance with the EU General Data Protection Regulations, individuals whose information is processed by Community Thrive are granted the following rights:
• Right to object
• Right to erasure

• Right to data access and portability

• In accordance with these rights, at any time you may request to:
  • Have your information removed from our records
  • Prevent us from processing your information
  • View and have access to your information
  • Modify or update your information

• To stop receiving email communication click the ‘Unsubscribe’ link at the bottom of any email communication from Community Thrive.

By using the Community Thrive app you consent and submit to the exclusive jurisdiction of the state and federal courts sitting in the County of Alameda in the State of California, and waive any jurisdictional, venue, or inconvenient forum objections to such courts.
